I have been a member for a month. The best part of this plan is the personal contact with the doctor and dietician. The doctor is extremely knowledgeable about obesity and works with you to find the best medication. The dietician texts me daily, and responds, day or night, to my messages.

Life Changing. I've been in the program for about 6 months and have lost over 60 pounds. The program is personalized to address your specific needs, likes, and dislikes. Unlike most weight loss programs, you work in collaboration with a doctor and dietician; which allows for long term and optimal success.
Julia and Dr Simpson go above and beyond. I have tried several programs like this and have never received the level of care that they offer. Danielle is amazing as well so really just the entire Form team has been wonderful!
I have had great success with form health, doctor and nutritionist are great. Have lost 10 lbs. in 6 weeks ,started during holidays. I have tried every diet and don't lose weight easily. The support has been great and I can for the first time in 7 years see my self getting to my goal to lose 20 more lbs. Its been a long 7 years of the scale barely moving. Give them a try they are great.
This has been the best weight loss program that I have tried. I have tried others, and been stuck at losing 10=15 pounds. I have lost over 30 pounds since October with Form and have been able to keep it off!! The support with the nutritionist and the medical doctor have been great. I have received useful information from them about making small changes to my eating habits and my exercise routine that have been very beneficial. I could not be happier!
I’ve tried many fad diets and shakes but it never lasted. Joining Form Health was the best decision I’ve ever made. I lost weight and learned how to choose healthier options that are realistic for me and my life. I cook ethnic food for myself and my family; it’s an important part of our life and our heritage. Through the program, I don’t have to force myself to eat pre-packaged low calorie food. With information that I learned, I can modify what I cook, and think intentionally about serving size and how to build a plate that works for my goals. Form Health taught me to make better decisions about what I eat. It’s ok to satisfy my cravings once in a while, and I have learned to forgive myself as long as it is a conscious indulgence. This is the first time I haven’t given up on a weight-loss program. It’s not a fad diet but a lifestyle change without changing who I really am, and that’s what makes it work, because I love who I am, and I love Form Health!
This is an amazing program if you want to lose weight and get healthy, but not stress too much. What worked for me he best is that I had an actual person to send my meals to, ask questions, and have friendly chats once a week. She would always listen to my challenges and make creative suggestions that worked for my personal lifestyle. For example, when I complained that I can’t get out of habit of snacking while watching Netflix after kids go to bed, Julia helped me come up with an idea of a tray of chocolate-covered strawberries in the freezer as a healthier substitution. I lost 10 pounds in this program, and got many healthy foods in my daily menu that I actually like.
This is a great program and very effective in helping you meet your goals. The app is easy to use. It allows for a easy way to communicate with the program and provides a quick method for you to lookup all the information they provide. Juila is great, knowledgeable and helps keep you accountable. I was able to meet my goal in 3 months and have changed the way I look at eating. It is definitely worth it!
I had work with Julia Axelbaum before I tried the new program. Being that I had experienced with her she had given me tips on what I needed to improve, how to cut out unneeded calories. This was easy to follow and being in constant contact with her and her co workers was a huge plus. When it comes to the Metro East area of St Louis Mo I would be glad to join.
